2 local health centers earn patient-centered recognition

Two community health centers have earned national recognition designed to improve their relationship and care of patients. 

The center at Donegan Elementary School in southside Bethlehem is now a National Committee for Quality Assurance Patient-Centered Medical Home Level III Recognized Practice. 

The Vida Nueva center located at Casa Guadalupe Center in downtown Allentown was upgraded to a Level III Recognized Practice from its previous Level II. 

The NCQA Patient-Centered Medical Home is a model of care that puts patients at the forefront by building better relationships between clinical care teams. The goal is to improve the quality of care and the patient experience.
